Payment Options

I am currently an In-Network Provider for:

FEES AND PAYMENT FOR SERVICES

Initial Intake Psychiatric Diagnostic Evaluation, 90791.          $275.00

Individual, 50-55 minutes, 90837                                                   $225.00                                        

Individual Counseling, 40-45 minutes, 90834                            $200.00

Individual Counseling, 30 minutes, 90832                                   $175.00

Full payment is due at the time of your session. Any balance due will continue to be due until paid in full.  If you do not have health insurance or are unable to pay, please inform me.  I offer payment plans or a sliding scale depending on individual circumstance.  

The practice requires that you keep a valid credit or debit card on file. This card will be charged for the amount due at the time of service and for any fees you may accrue unless other arrangements have been made with the practice ahead of time. It is your responsibility to keep this information up to date, including providing new information if the card information changes or the account has insufficient funds to cover these charges.

You may pay for sessions using cash fee-for-service, HSA, or your health insurance.

INSURANCE REIMBURSEMENT AND FILING POLICY

Please remember your insurance is a contract between you, your employer, and the insurance company or between you and your insurance company directly. Journeys Within Counseling, LLC is not part of that contract.  Please contact your insurance company to verify mental health benefits and determine your deductible, co-insurance, co-pay, and if you need authorization to receive treatment from the provider. Insurance companies require a diagnosis for mental health services. To receive reimbursement from insurance, a diagnosis and any requested information must be supplied.

CANCELLATION AND NO-SHOW POLICY

The office policy is a 24-hour notification on all cancellations or rescheduled appointments. No show or late cancellation fee is $45. This fee will be charged to your Debit/Credit card on file. If you do not have payment information on file, this fee must be paid before another appointment can be scheduled. Insurance companies cannot be billed for this fee.

BANK FEES/NFS CHECK RETURNS

If a payment is returned for non-sufficient funds, and your transaction is declined (or check returned), a $45.00 will be charged per incident. This fee must be paid before another appointment can be scheduled.

You may pay for sessions using cash fee-for-service or your health insurance.
